You didn't get the slide aligned with the frame rails correctly. It appears the slide was not flat (i.e., tilted up at the muzzle) on the frame when you reassembled it. Disassemble the pistol and start over making sure the slide is aligned correctly and flat on the frame rails.

Yep- I agree- the slide is definitely not in the groves.

 

Yup I put it on wrong. It's still stuck. Any advice?

 

I ended up fixing it. I had to press on the side release button and the trigger then push the slide forward. Pressure then fixed.

Never force it. If it's aligned it should slide right in without much effort.

 

I had a similar situation with a Shield Plus and had to send it back to S&W to fix. The slide/frame junction was good so I suspect it was a misaligned recoil spring rod. S&W did not enlighten me as to where the problem lay. The sear disconnect lever would not move to allow a magazine insertion so that required factory attention too. I don't own that gun anymore.

 

Tap the slide backwards with SOFT Hammer while depressing release,Its happened to me once or twice when not focusing.
