Is there a holster out there with a retention strap or flap for the CIA hammerless revos? I'm considering one, but wouldn't feel comfortable at times with an open top holster where the gun could fall or bounce out. IWB or pocket carry would be out of the question at these times.

A good quality IWB holster will not need a retention strap to hold the gun in place. The fit does that. I've had cheap holsters that needed the strap (they got returned) but even inexpensive OWB holsters like the Don Hume JIT will hold the gun just fine.

Check out the holsters from http://www.miltsparks.com or http://www.mitchrosen.com . Yeah, they're a might expensive and you have to wait a few months on delivery, but it's all worth it for quality gun leather. Qualilty leather (and belts) fit the user as well as the gun and make all day carry much easier.

An example from the Milt Sparks line. I got a PMK for a 3" N frame for a woods gun. After a quick break-in period, to test the holster's holding ability of the gun, I held the holster with gun upside down over my bed (unloaded gun) and shook it repeatedly up and down. The gun didn't move any.

If you decide to order a Sparks but don't want to long wait, try here: http://www.lightningarms.com/ I've dealt with them a couple of times and have yet to hear of someone having poor service from them.

Another site to order good holsters from (not as nice as Sparks but still good) is http://www.gunnersalley.com/ Once again, I've dealt with them and had good service from them. They do have a good return policy and I have used it (a holster that just didn't fit my body).

I just use one of the Uncle Mike's Sidekick holsters for snubby revolvers. It has the retention strap, and is less than $20. I don't carry the 650 much, however. If I did, I might look into something else.
