I wish I thought that was a rare occurance. A friend of my LEO son, was suiting up for a raid, in the department armory "he was a member of the tactical team", not only did he shoot himself in the hand,(can we all say, know where the muzzle is pointed) causing his having to leave the work. He caused hearing loss in several other officers so severe that they could no longer work. That department lost about 5 officers for one stupid mistake.
