I made my iwb crossbreed style holster from items purchased at Springfield Leather. It was easy and actually kind of fun...planning on making another for my pt738.

Does it come with directions? I never even thought of making my own holster. Also, I have a Judge with a laser/light combo mounted under the barrel. Could I make one for that?

Get a kit w/Kydex material you can heat to fit your weapon from Gun Holsters - IWB Holsters - Concealed Carry Holsters - Gun Belts - price is reasonable. They may even have a pre-mold for your weapon with laser/light combo. I have a MOB for my PT24/7 made from a kit and it's a great holster.
